What you'll do

  • Provide individualized care to residents according to each resident’s plan of care.

  • Assist residents with activities of daily living (ALDs) including bathing, grooming, and dressing.

  • Assist with mealtime activities, including feeding residents as needed.   

  • Assist residents with mobility, including transferring and positioning them safely.

  • Monitor residents’ health status and vital signs and report any changes or concerns to the nurse.

  • Engage residents in social and recreational activities to promote overall well-being.

  • Maintain safe, clean, and organized living environment for residents.

  • Perform other duties as required to provide exceptional care to each resident.

What you need

  • Active certification as a Certified Nursing Assistant in state of practice.

Certified Nursing Assistant Weekends Only 7-3 and 11-7

  • Delivers basic nursing care, including activities of daily living (ADLs) and

restorative program(s) based on individual patient needs and plan of care.

  • Ensure compliance with company policy as well as federal, state, and local regulations.

  • Thoroughly document all care provided, patient response to care, and patient refusals of care.

  • Accurately measures and documents vital signs, height, weight, and other necessary patient data.

  • Takes ownership of patient hydration rounds to ensure all patients maintain adequate hydration.

  • Provide meals per patient preference (in-room/dining room); assist with meals as needed based on individual patient care plan and monitor / document patient intake.

  • Respond to call bells, signal lights, intercoms, and patient requests for assistance in a timely manner.

  • Immediately communicate changes in patient condition to licensed nurse.

  • Utilizes appropriate individualized behavior response techniques under the direction of the licensed nurse.

  • Safely transfers and transports patients using proper body mechanics and appropriate safety equipment.

  • Operates all equipment / devices according to guidelines / operational instructions.

  • Complete frequent room rounds to ensure clean, orderly, and safe patient environment.

  • Participates in the admission / discharge / transfer process.

  • Perform properly cleaning and disinfection of equipment and devices in accordance with company standards and guidelines.

  • Review and investigate patient, family, and staff concerns; take action to resolve and prevent reoccurrence.

  • Participates in training / precepting of new staff.

  • Attend staff meetings / in-services as needed.

  • Completes all required trainings in a timely manner.

  • Performs other duties as necessary for the successful operation of the healthcare center.

Staff Development Coordinator, RN

The Staff Development Coordinator (SDC) is responsible for providing new hire orientation, annual follow up training, and education in-service programs as needed for all staff. The SDC plans, organizes,

implements, and coordinates the healthcare center’s nursing staff education programs. The SDC evaluates

the educational needs of the healthcare center to ensure on-going continuing education and adherence to

applicable required in-services. The SDC identifies, assesses, and implements in-service programs as

needed regarding the satisfactory performance of clinical skills and knowledge.

Recreation Assistant at Long Term Skilled Facility


The Recreation Assistant supports in the coordination and implementation of the center’s on-going schedule of meaningful recreational activities designed to meet the physical, mental, cultural, spiritual, emotional, and psychosocial interests of each resident in accordance with company policies and procedure as well as local, state, and federal guidelines and regulations.

 Maintains organized, clean, and safe work area, including activity rooms and common areas.
 Manages time, projects, and records effectively.
 Attend and actively participate in meetings as requested.
 Ensure all mandatory training is completed on time.
 Assists with new-hire orientation, training, in-services, and staff meetings as needed.
 Participates in the department’s Quality Assurance and Process Improvement (QAPI) program and assists in resolving any issues identified as part of the   QAPI process.
 Reports all instances of suspected abuse or neglect or other concerns to management.
 Recognize abnormal signs and symptoms, which may signify a physical change in any resident’s condition that could require nursing intervention; report   the same to charge nurse immediately.
 Maintain confidentiality of all resident information.
 Performs other duties as necessary for the successful operation of the healthcare center.
